Sexual Healing

I was asked by O to write about what medicine is observing in regards to changing sexual morality these days. I’ve been in the Anesthesia since the eighties.  I no longer work at Grady, Atlanta’s inner city hospital/Gun and knife club. I have graduated to the suburban northern hospitals.   Always thought we in healthcare had a unique perspective into changes in society.  After all, we get the health history lowdown on all our patients and our clientele is mostly naked.  We find out things people wouldn’t tell their priest during a confessional. We are often the first to see societal changes in occupations, unemployment, sports related injuries, trends in pubic hair styles, etc.  We Operating Room folk are on the front lines observing our ever changing society.  A couple years ago I started seeing a disturbing trend involving teenage girls.

One of the facilities my Anesthesia group covers is a small outpatient gynecologic surgery center that is owned by a group of OB/GYNs here in Georgia.  We do almost all sorts of GYN procedures, except for elective abortions.  Lately, we have been seeing teenage girls (as young as 15) having large gaping rectums.  Now, we have always seen this in homosexual men…..but never in teenage girls.  We attribute this sudden increase to anal sex.  According to ABC news, anal sex among teens is on the rise. This increase in anal sex has greatly contributed to the increase of HIV and STD infections.  The tissue of the anus and rectum is very vascular and delicate, and tears easily.  These tears will allow infected fluids into the blood stream more easily, thus facilitating the spread of STDs.  Teenagers are not taught about anal sex in High School, or by their parents.  They are under the mistaken impression that anal sex is safer…. (that STDs and pregnancy cannot occur during anal sex) ….so they are less likely to use condoms.  Anal sex is more common among girls who have pledged to save their virginity for marriage because they believe anal sex is not” really sex”.

Another phenomenon we are seeing is the surge of Labiaplasties.  The group that most often has this procedure is young white women ages 16-25.  The diagnosis we are getting for this procedure is “painful exercise and bike riding”.  Apparently, girls these days have labia so large they can’t ride their bikes to and from school. (This medical condition must be new because we old gals never had a problem riding our bikes back in the day).  I had a patient last week who had this procedure.  She was an 18 year old girl who was headed off to NC State.  She told her mom and dad that she suffered from painful bicycle riding and convinced them to pay $4000 large up front for this procedure.  She, like many of her peers, had a gaping anus as well.  We are currently doing two of these surgeries per week in our small center. (Personally I think this cash up front surgery is supplementing lost income from Obamacare….I’m just sayin’).

The symptoms of painful bicycle riding, coupled with a gaping anus, seems to be accompanied by the bizarre use of anal bleaching cream and the desire for anal albino ism.    So far this “epidemic” seems to affect mostly rich girls from the UMC.  It could be, however, that the poorer girls have to ride their bikes in silent agony because mom and dad cannot afford $4000 for the corrective surgery.  This epidemic is so new that the CDC doesn’t even know about it yet!

As much as I like to make fun of Labiaplasties, anal bleaching, and gaping anus’ (anuses?  annii?) …..the real problem of porn use among teenagers is no laughing matter.  Girls, as young as 12, are getting their first lessons about sex from porn.  The porn stars like Jenna Jamison, who’ve had labial reductions and anal bleaching, are what teenage girls are emulating.   Porn stars like James Deen are the new teenage heart throbs. Studies show most porn is viewed on small screens like a smart phone.  In fact, in the U.S., 52% of porn use is on a cell phone.  Our children are introduced to porn when we give them their first phone. We know constant porn use can cause changes in brain function, but imagine the damage it can cause to an immature brain.  Such bad parents Americans have become!  We pay for our daughter’s breast augmentations and Labiaplasties, and then send them off to college…. confident that their cell phones and their “virginity pledge” will keep them out of trouble. LOL!!!!

HPV (Human Papilloma Virus) is the most common sexually transmitted disease in the U.S.  It’s estimated that most of Americans are infected with HPV.  HPV causes genital warts (also called genital condylomas) and cervical and penile cancer.  One of the biggest genital condyloma that I have seen recently was a teenage girl whose grandmother (guardian) refused to take her to the doctor to remove her grapefruit size genital condyloma in order to teach her a lesson.  Only when she was not able to attend school did her grandmother allow her to have it removed.
Lately, we are seeing an emergence of oropharyngeal cancers caused by HPV.  If you remember, Michael Douglas made the news a while back after he announced he acquired tongue cancer from oral sex.  Oral HPV is acquired through oral sex.  Men account for 70% of all oral HPV cancer cases.  In the past, throat cancers required radical neck surgery.  However today, throat cancers have been responding to newer chemotherapy treatments and have a greater survival rate. There is no treatment for HPV. Condoms don’t protect against HPV entirely….they just reduce the chance of transmission.  Once you get it….you’ve got it for life.  New vaccines like Gardasil protect against HPV strains 16 and 18, which are responsible for most types of cervical cancer and genital warts. It is recommended that boys and girls get vaccinated before the age of 12 so their immunity will set in before their first sexual encounter. The vaccine is not as helpful for African American women who are more frequently infected with HPV sub-types 35, 66, and 68.  Pharmaceutical companies are continuing their research for vaccines that include more sub-types of HPV. I strongly suggest to everyone that they have their kids vaccinated.

Chlamydia is an STD that is caused by the bacteria Chlamydia Trachomatis.  It’s the most reported STD in the U.S.  Chlamydia is easily treated with antibiotics.  The only way to get Chlamydia is to have sex with someone with Chlamydia, or pass it along to your child at childbirth.  The problem with Chlamydia is that it is insidious.  You don’t know you have it until it does some damage.  It affects the reproductive and genitourinary systems in both men and women.  In women it is the #1 cause of Pelvic inflammatory disease, infertility, and ectopic pregnancies.  This is what we commonly see when women come in for laparoscopic infertility workups.  When men have chlamydia they are usually asymptomatic. When men do have symptoms, they include penile discharge, testicular swelling, burning urination, and occasionally infertility. Men who asymptomatic can pass it to their partners without even ejaculating. Chlamydia is most often seen in sexually active young people ages 14-24.  It is 5 times more prevalent in black women than white women.

The southeastern U.S. has long been lauded for its inferior school systems and obesity rates.  Well, it appears we have a new feather to stick in our baseball caps.  We are now the STD hub of the whole USA!  I can explain away the obesity because of our passion for biscuits and sweet tea……and the school systems ‘cause we rather be fishin anyway….but the STDs in the Bible belt were a mystery. On further examination, it appears that African Americans have substantially more STDs than their white neighbors.  It is estimated that half of the African American population lives in the southeast, which would account for the higher STD rates.  As said before, black women have 5 times more chlamydia than white women.  African Americans have a 30 times higher rate of syphilis and gonorrhea than Whites. I have no stats on the new and improved “super drug resistant gonorrhea”….that’s a wait and see right now.  In my state of Georgia, African American women received the most abortions.  Out of 34,500 abortions in 2010, the most common recipient were black women–18-24—unmarried—college educated. Two things contribute to pregnancy and STDs………….lack of protection and promiscuity.  I initially attributed the problem to possibly not using condoms, but studies show black men use more condoms that whites.  So that left promiscuity as the problem.  As it happens, in 2013 only 32% of black men and 26% of black women were married. That’s compared to 44% for white men, and 51% for white women.  This new phenomenon that “Marriage is for White People”  may have caused an increase in the number of sex partners in the black community, thus increasing their risks for exposure to more STDs.  An article in the Washington Post by Joy Jones said that marriage in the black community was down 34% since 1970.  According to this article, black women were the least likely to marry. A sociologist was quoted to say that a black child was more likely to grow up in a two parent household during the days of slavery than they were today. It went on to say that black women want to marry in their early twenties, but black men do not.  So black women are more likely to go to college and have babies solo.  When black men are ready to marry in their 30s, but black women already have their children and their careers set.  It is a dire situation for traditional family homes.  The author shows that whites are beginning to go in that same direction.  Traditional families may soon be a thing of the past.

So to summarize, the only way to minimize your risk of acquiring a sexually transmitted disease or infection is to limit your sexual contact to long term monogamous relationships and use condoms until you and your partner have tested safe.  You don’t have to be promiscuous to get an STD…….. (one time with the wrong person will do it)……but promiscuity greatly increases your odds.  So if you meet a PUA who just F***ed his way through the former Soviet Union….or these gals from The Frisky……my advice to you is RUN!!!!!
